@jackjeanckl6608 since you already addressed all the software possibilities, this sounds like a failed camera. At this point I'd suggest that you replace the camera. Since Ifixit does not yet have a guide for this, use something like this video It is not specific to the camera but should help you to replace yours. Something like this guide should also work for this.
The cameras are readily available at many online stores.
While you work on your phone, take lots of pictures. Once you are done with it, take those pictures and create a guide for iFixit.
