The most common source of this issue is debris in the headphone jack causing the sensor inside to continually detect headphones. This in turn forces the phone to turn off the receiver earpiece and mic. The most common source of this debris is pocket lint, BTW.
You can also test this by playing a track in iPod mode - if it doesn't play thru the speaker, and the volume control indicates headphone adjustment with no headphones connected, then the sensor is being triggered.
Try blasting compressed air into the headphone jack in a series of short blasts. The debris can also sometimes be cleared by repeatedly inserting and removing headphones from the jack.

Have you taken your iPhone apart? If so, it could be that ribbon #3 is out of place- that one corresponds to the earspeaker.

How far is the #3 cable supposed to go into the phone? it looks as if there is a white line with an arrow pointing to it. Should it go in that far, or only until the "light" colored brass in all the way in?
