I replaced a cracked glass with the combo replacement (glass preassembled in frame) unit. Everything went perfectly but after installation there is a dead area about a 1/2" wide and 1" long directly above the button on the face of the device. Is this a defective digitizer?

You could recheck the cable that connects it to the logic board and make sure it is in there well. If it is and it still doesn't work than it is probably a defective digitizer.
