This usually means the screen assembly is damaged and needs replacing.

It is a straightfoward repair. You can do it by following this step-by-step guide. The guide also includes links to the needed tools and good quality parts. Don't go cheap, avoid unknown parts sellers on Amazon and eBay. You'll end up spending more to replace defective parts and repeating the repair. Always go for good quality parts and reputed sellers that stand behind their products and offer a good warranty. Contrary to what you read in other posts, for the iPhone 5C, the outer glass and LCD are fused together and should be purchased as an assembly. Some people who have the right equipment can detach the LCD from the outer glass and replace each alone (usually done if the LCD is good and the outer glass is broken, which is NOT your case since the LCD AND touch are damaged.. in which case you just buy a new screen assembly).
