My iphone5c connects to wifi fine, but after about 5 minutes it disconnects and does this every 5 minutes, it has been like this for a week now, also the time and date keeps changing on my phone constantly, i have tried restarting the network settings, restarting the phone, and still nothing is helping my phone go back to fully working, what is wrong with my iphone?

if all else fails, try a Restore of iPhone with iTunes on computer.
