For those of you with this problem you have a few things you can try to alleviate the issue:

Hard resetting iPad:

1) Ensure the iPad is ON

2) Hold the Power button and the Home button simultaneously until you see an apple logo appear.  (ignore the slide to power off as it will reset while holding the buttons)

If that does not fix the issue, the next thing to do is to open up your iPad (I know it sucks) and to check all of your connections, especially the double connector leading to the digitizer on the bottom left corner of the iPad.

Ensure that it is fully seated within the connector clamps, and also ensure that when you reapply the glass that you are NOT pinching that cable, yes it is going to cover the cable, but if you have it folded or kinked it will give you those type of errors.

Also when purchasing new glass for iPads, keep in mind Apple does not make replacement parts and most if not all glass bought will be from a 3rd party.  I recommend looking on eBay for a seller that has sold at least 100 of them just to ensure you get a quality piece.

Your last step is to purchase a new glass + digitizer.
