How can I replace a cracked iSight/rear camera lens cover?

I dropped my iPhone 6S Plus (A1634) and cracked the glass cover that sits on top of the rear camera.

1. How is the rear lens cover attached to the frame? I've seen a lot of conflicting information out there - some people say it's glued on and you can simply hit it with heat gun and pry it off. Others say you'll have to disassemble the phone and poke it out.

2. If I do have to disassemble it there seems to be a metal bracket that I have to remove which gets warped in the removal process. iFixit doesn't sell this bracket. Any suggestions on where I can buy it?

Tried applying gentle heat to the exterior side of the lens cover with a warm flat metal blade to no avail. I also tried tapping the cover out from the inside out using a cylinder that fits the metal ring surrounding the glass, also to no avail. I'm guessing it is glued in with a strong adhesive.

iFixit now carries the camera lens cover, which I purchased, but there is no tutorial on replacing this lens cover. I'm hoping the crack is too close to the lens to affect photographs, since the depth of field of the lens is longer than the distance of the cover from the lens. I'll add to this post after this repair is completed and I take some photos.

Done it plenty of times.

I use 80°C - 120°C heat blasting onto the rear camera lens, with the rear camera removed then poke it out. If it is cracked I take my time prying off the cracked glass from the rear camera lens.

Then to put the new one on I either use the circle adhesive that was already pre-applied for the camera or just a bit of B7000 liquid glue and let it set over time.
